Exacerbate needs a problem already there. You cannot exacerbate a good day; you exacerbate a shortage, an injury, a delay. The Latin behind it is acerbus, bitter — to exacerbate is to make a thing more bitter than it already was. Watch the drawing: the fire is small and a man is cold. Then the newspaper goes on, and on, and the little fire becomes the thing with a face. Nobody in the picture set out to do harm, and that is the ordinary case. The word points at what was added, not at who started it, which is why reports keep the cause and the exacerbation apart.
