A callus is the hard skin that grows where something rubs — a guitarist's fingertips, a gardener's palm — and callous is that same thing happening to feeling. This matters, because it explains what the word is actually accusing somebody of. Nobody is born callous. The skin thickened, one rub at a time, usually for a reason: too many bad cases, too many years on the ward, too much of the thing that used to hurt. The hard part is that the hardening works. The callous person does feel less, exactly as intended, and the cost is paid by whoever needed them to feel it.
